• Your email address, if you choose to give it
• Preferred days, insurance, and whether another doctor referred you
• Any short message you write

Our website host may also keep standard server logs, such as IP address, browser type and pages requested, for security and reliability.

How we use it

We use what you send only to contact you and arrange an appointment.

How it is handled

Appointment requests are sent as an email to our office and are not saved in a database on this website. We do not use a third-party form service to collect them. The message is sent through our own office email account, so no separate delivery service keeps a copy of it. Once it reaches our inbox, it is handled like other office email.

Please don't send medical details

The form and email are not the place for medical information. Please don't include symptoms, test results or diagnoses. Call us at (214) 770-3610 and we'll talk with you.

Cookies and analytics

This website does not use advertising or tracking cookies, and does not currently use analytics. If that changes, we will update this page.

The Contact page embeds a Google Map of our office. When the map loads, Google may set its own cookies and collect usage data under Google's privacy policy.

We do not sell the information you submit through this website, and we do not use it for marketing.

If this site links to a scheduling service, patient portal or bill-pay page run by another company, that company's own privacy policy applies once you leave this site.
