Overview
About diabetes care care
Diabetes is a condition in which the body has trouble regulating blood glucose (sugar), either because it does not make enough insulin or cannot use insulin well. Over time, high blood sugar can affect the heart, kidneys, eyes and nerves, which is why steady, long-term care matters.
Dr. Nazeer cares for adults with Type 1 diabetes, Type 2 diabetes and prediabetes. Care includes insulin therapy and continuous glucose monitoring (CGM), with first visits in person at our Plano office and telehealth available for many follow-up visits.
What we treat
Diabetes care includes
Type 1 diabetes
An autoimmune condition in which the pancreas makes little or no insulin, so insulin must be replaced.
Type 2 diabetes
The most common form. The body does not use insulin effectively and may not make enough over time.
Prediabetes management
Blood sugar that is higher than normal but not yet in the diabetes range. An important window to act.
Insulin therapy
Choosing, starting and adjusting insulin, with a dosing plan that fits your routine.
Continuous glucose monitoring (CGM)
A small wearable sensor that tracks glucose around the clock and shows trends between finger-stick checks.

Questions
Frequently asked questions
What should I bring to my first visit?
A list of your current medications and doses, your glucose meter or CGM data if you use one, and any recent lab results done elsewhere. If you have a referral or insurance card, bring those too.
What is a continuous glucose monitor?
A CGM is a small sensor worn on the skin that measures glucose every few minutes and sends readings to a phone or reader. It shows patterns across the day and night that single finger-stick checks can miss.
Can I be seen by telehealth?
Your first visit is in person at our Plano office, where Dr. Nazeer examines you and builds your plan with you. Telehealth is available for many follow-up visits, and our office will let you know which type of visit fits.
Do I need a referral?
Some insurance plans require a referral to see a specialist and others do not. Check with your plan, or call our office and we will help you find out.
